This is a cursory evaluation. It may be helpful on very young patients or on others when there is a question of the feasibility of orthodontic treatment. Below are two forms that you can fill out before coming in for your appointment. 2) Exam Appointment
This is our most common initial appointment. A complimentary Panoramic Radiograph (x-ray) and computer photos are taken of your teeth and face to help the doctor determine: - Is orthodontic treatment indicated?
- When is the best time to initiate treatment?
- What treatment options are available?
- How long will the treatment last?
- How much will the treatment cost?
Below are two forms that you can fill out before coming in for your appointment. 3) Facial Pain Evaluation
This appointment is indicated on those patients whose primary concern is facial pain or TMJ symptoms. Radiographs of the TMJ are taken. Since extensive Doctor time is involved, a fee will be charged on any appointment not cancelled at least 24 hours in advance. Before coming in for you evaluation you can print and fill out the questionaire below. Our very capable staff will be glad to work with you to clarify your insurance coverage and help you receive the maximum allowed payment plans available. |
